Acne conglobata is a rare and severe form of acne. It is usually manifested in the form of lesions (comedones) that occur in clumps of two or three. Inflammatory nodules appear around these clumps of comedones. They slowly increase in size and then bust thereby producing foul smelling pus. Ulcers can form below these nodules which get joined to each other under the skin and make abscesses. These abscesses leave deep and irregular scars after treatment.

This form of acne is infectious and typically appears between the age of 18 to 30 years. It usually continues for many years; sometimes till the age of 40 years. It normally occurs to people with active acne infection; through it may also infect people already healed of acne. It occurs more in males than females and can ever happen to infants. It can happen at many places, such as the back, buttocks, chest, face, upper arms, shoulders and thighs.

The main medication to treat acne conglobata is the retinoid drug Isotretinoin. It is derived from Vitamin A and is marketed under the name Accutane. This medicine works by reducing the oil output of sebaceous glands present under the skin. It is available in the form of tablets and is usually prescribed for a 15 to 20 week course. A single course usually clears acne conglobata and gives long term remission to most of the patients. This medicine may be combined with systemic steroids, such as prednisolone, particularly if systemic symptoms are present.

Tretinoin is another retinoid drug that helps treat acne conglobata by eliminating lesions. It also reduces the tackiness of keratinocytes on the skin and makes it easier to remove them. The doctor may also prescribe antibiotics, such as tetracycline or erythromycin, either alone or in combination with isotretinoin or prednisolone. Dapsone, a leprosy drug, is recommended for treatment of resistant cases though its use should be cautiously monitored.

Surgical techniques are exercised as the last option to treat acne conglobata. Large hemorrhagic nodules can be removed by surgery. Sometimes, surgical elimination of connected large nodules may be helpful. Intralesional triamcinolone or cryotherapy can also be performed.

Another way to treat acne conglobata is through carbon dioxide laser and topical tretinoin therapy thereafter. Injecting triamcinolone into the acne nodules may also be suggested by the doctor. Vitamin and mineral supplements that contain vitamins A, E, C, B6 and Zinc also help fight acne conglobata. They boost your immune system that eliminates acne causing bacteria.

You should also take a balanced diet that comprises of healthy food items, such as seeded fruits and organic vegetables. Additionally, you should exercise regularly as it improves your blood circulation.
